Forms field options don't work

If you select a 5 star field in the grid, it doesn't carry over on the form. it only shows a drop down with the #s 1-5 to select in the form. Not very visually pleasing to customers. Same with the YES/NO and checkbox for multi select defaults to a drop down.
Also, if you added a default, it will not show that as the option (such as defaulting to yes for yes/no).

Forms currently show yes/no field as "no" even if the default in the table is set to "yes". Small thing but it would prevent some confusion on the forms we use for client submission.

I have a client submission form created from a client information table. Referral agencies fill out the form which then populates the usual fields in the table (name, DOB, etc). One of the fields is a "Yes/No" which identifies that the client being referred is their own guardian "Yes" or "No". Our clients are most commonly their own guardians and in the Table this defaults to "Yes". However, when presenting this in the form, this field presents as "No". I would prefer to have this show as "Yes", matching its default state from the Table.
Also -much later "wish list" stuff... I would like to be able to use my own text for Yes/No fields such as "To be done/Completed".

- What specific forms are affected by this default option issue?
- Is the default option issue occurring across all platforms (web and mobile) or specific to one?
- Are there any other fields in the forms that are not reflecting the default values from the table?

It would be great to be able to use the dropdown options and 5 star option in forms. Currently, it works in the grid but not on the form.
